I am trying to find the correct main stream url. The only one that I can get to work is the substream.
http://192.168.1.221/cgi-bin/mjpg/video ... &subtype=1
When I change the subtype to 0, I get prompted for my userid/password but I get a page isn't working error and it keeps prompting me for my userid/password again.
Anyone have any suggestions?

The code you use is for the sub stream of the camera and the sub type should be 1. The mainstream you can only get with the following RTSP command but it will not work with Browsers. You need a RTSP player like the VLC media player for this.
